Alcohol & Drug Policy  Alcohol & other Drugs: Administrative guidlines for the Ramapo GryphonFootball Player. This sheet must be signed by both you and your parent/guardian and turned in to Coach Vitolo. |

Expectations of the parent/guardian
Besides expectations for the players we also have expectations for the parents/guardians. This sheet must be turned in to Coach Vitolo along with the other sheets. Your parent or Guardian must sign and date it. |

Goals The Ramapo Gryphon Goal Sheet is a form to fill out to see not only what team goals you have but personal ones as well. This should be filled out and turned into Coach Vitolo.

Housekeeping Contract Each player is responsible to keep not only a neat/organized locker but a neat lockeroom as well. This sheet must be signed by you and turned into Coach Vitolo |

Membership Rules for being on the Team Being a member of the Ramapo Football Team is a very special thing that requires committment from a very special person. This form outlines the expectations that the coaching staff has for it's players. It must be signed by you and your parent/guardian then turned in to Coach Vitolo. |

More Information about YELLOW CARDS Every player must be issued a yellow card to participate on the team. Besides the permission slip and physical you may have to do get this done also. |
Parent Permission Slip  To participate on the team this permission slip must be filled out and signed by your legal guardian/parent. It then must be turned in to Smitty the Athletic Trainer. |
Sports Physical Form for Private Doctor  The school offers free sports physicals to the student athlete. However, if you choose to use your own Doctor, he/she must complete this form. This form should be tuned in to Smitty the Athletic Trainer or the School Nurse. |
